Welltower Inc., incorporated on April 4, 1985, invests with seniors housing operators, post-acute providers and health systems to fund real estate and infrastructure. The Company invests in seniors housing and healthcare real estate. It operates in three segments: triple-net, seniors housing operating and outpatient medical. Welltower, a real estate investment trust (REIT), owns interests in properties concentrated in markets in the United States, Canada and the United Kingdom, consisting of seniors housing and post-acute communities, and outpatient medical properties. The Company's primary objectives are to protect stockholder capital and enhance stockholder value.

Triple-Net

The Company's triple-net properties include independent living facilities, independent supportive living facilities (Canada), continuing care retirement communities, assisted living facilities, care homes with and without nursing (the United Kingdom), Alzheimer's/dementia care facilities, long-term/post-acute care facilities and hospitals. The Company invests primarily through acquisitions, development and joint venture partnerships. The Company's properties are primarily leased to operators under long-term, triple-net master leases. Its properties include stand-alone facilities that provide one level of service, combination facilities that provide various levels of service, and communities or campuses that provide a range of services. The Company's independent living facilities and independent supportive living facilities are age-restricted, multifamily properties with central dining facilities that provide residents access to meals and other services, such as housekeeping, linen service, transportation, and social and recreational activities.

Continuing care retirement communities include a combination of detached homes, an independent living facility, an assisted living facility and a long-term/post-acute care facility on one campus. Assisted living facilities are state regulated rental properties that provide the same services as independent living facilities, but also provide supportive care from trained employees to residents requiring assistance with activities of daily living, including management of medications, bathing, dressing, toileting, ambulating and eating. Care Homes with Nursing are licensed daily rate or rental properties where the individuals require round the clock nursing and medical care. Care Homes are rental properties that provide the same services as the United States assisted living facilities. Long-term/post-acute care facilities include skilled nursing/post-acute care facilities, inpatient rehabilitation facilities and long-term acute care facilities. Skilled nursing/post-acute care facilities are licensed daily rate or rental properties where the individuals require round the clock nursing and medical care.

Seniors Housing Operating

The Company's Seniors housing operating properties include a range of facility types, including independent living facilities and independent supportive living facilities, assisted living facilities, care homes and Alzheimer's/dementia care facilities. Its properties are primarily held in consolidated joint venture entities with operating partners.

Outpatient Medical

The Company's outpatient medical properties include outpatient medical buildings. The Company leases its outpatient medical buildings to various tenants and provides varying levels of property management. The outpatient medical office building portfolio consists of healthcare related buildings that include physician offices, ambulatory surgery centers, diagnostic facilities, outpatient services and/or labs. The Company has affiliation with health systems. The Company's outpatient medical building portfolio is affiliated with health systems (with buildings on hospital campuses or serving as satellite locations for the health system and its physicians).
