DaVita Inc., formerly DaVita HealthCare Partners Inc., incorporated on April 4, 1994, operates one division: DaVita Kidney Care (Kidney Care). The Kidney Care division consists of the Company's United States dialysis and related lab services, its ancillary services and strategic initiatives, including its international operations, and its corporate administrative support. The Company's segments include U.S. dialysis and related lab services and Other-Ancillary services and strategic initiatives. Its U.S. dialysis and related lab services line of business provide kidney dialysis services in the United States for patients suffering from chronic kidney failure, also known as an end-stage renal disease (ESRD). In addition, as of March 31, 2019, the Company operated or provided administrative services to 243 outpatient dialysis centers located in nine countries outside of the United States.

U.S. dialysis and related lab services

The Company's U.S. dialysis and related lab services business is a provider of kidney dialysis services for patients suffering from ESRD. As of December 31, 2018, the Company operated or provided administrative services through a network of 2,664 the United States outpatient dialysis centers in 46 states and the District of Columbia. As of December 31, 2018, it also provided acute inpatient dialysis services in approximately 900 hospitals and related laboratory services throughout the United States. The Company provides dialysis and related lab services, including Outpatient hemodialysis services, Hospital inpatient hemodialysis services, Home-based hemodialysis services, ESRD laboratory services and Management services.

As of December 31, 2018, it had provided hospital inpatient hemodialysis services, excluding physician services, to patients in approximately 900 hospitals throughout the United States. It renders these services based on a contracted per-treatment fee that is individually negotiated with each hospital. Its outpatient dialysis centers offer certain support services for dialysis patients preferring and are able to perform either home-based hemodialysis or peritoneal dialysis in their homes. Home-based hemodialysis support services consist of providing equipment and supplies, training, patient monitoring, on-call support services and follow-up assistance. Registered nurses train patients and their families or other caregivers to perform either home-based hemodialysis or peritoneal dialysis.

The ESRD laboratory services have consisted of two separately licensed, clinical laboratories which specialize in ESRD patient testing. These specialized laboratories provide routine laboratory tests for dialysis and other physician-prescribed laboratory tests for ESRD patients which are integral components of the overall dialysis services. Its laboratories provide these tests predominantly for its network of ESRD patients throughout the U.S. These tests are performed to monitor a patient’s ESRD condition, including the adequacy of dialysis, as well as other medical conditions of the patient.

Ancillary services and strategic initiatives businesses

The ancillary services and strategic initiatives consisted primarily of pharmacy services, disease management services, vascular access services, clinical research, physician services, direct primary care and its international dialysis operations and relate primarily to its core business of providing kidney care services. Its Ancillary services and strategic initiatives consisted of Pharmacy services, Disease management services, Vascular access services, Clinical research programs, Physician services and Direct primary care. DaVita Rx is a pharmacy that specializes in providing oral medications and medication management services to patients with ESRD and other chronic diseases. VillageHealth provides integrated care management services to health plans and government programs for members/beneficiaries diagnosed with ESRD and/or chronic kidney failure. Lifeline provides management and administrative services to physician-owned vascular access clinics that provide vascular services for dialysis and other patients. Lifeline is also the owner of nine vascular access clinics and also owns one vascular access clinic. DaVita Clinical Research (DCR) is a provider-based specialty clinical research organization with a spectrum of services for clinical drug research and device development.

DCR uses its applied database and healthcare experience to assist in the design, recruitment and completion of retrospective, prospective pragmatic and clinical trials. Nephrology Practice Solutions (NPS) is an independent business that partners with physicians committed to providing clinical and integrated care to patients. NPS provides nephrologist employment opportunities in select markets and offers physician practice management services to nephrologists. These services include physician practice management, billing and collections, credentialing, coding and other support services. NPS also provides nephrology recruitment and staffing services. Paladina Health is a healthcare services company that operates membership-based primary care clinics through employer-based onsite and near-site clinics. The clinics offer patients personalized access to primary care physicians, including unlimited visits and same-day or next-day appointments. Physicians focus on clinical outcomes and patient satisfaction.
